Academic Case Manager- Gravity

Purpose Statement

The job of Academic Case Manager- Gravity is done for the purpose/s of reviewing student academic performance and intervening when problems are identified, with the goal of providing support for students to stay in school and graduate which includes monitoring and advocating for students, referring services, providing ongoing feedback and encouragement, and emphasizing the importance of staying in school; and developing, coordinating, and providing a high school and beyond plan for students receiving services through the program, in partnership with school personnel, family members, and community service providers.

This job reports to Director - Student Support and Engagement/Gravity or Designee

Essential Functions

  • Collaborates with a variety of groups and individuals (e.g. juvenile justice and court staff, district personnel, community groups, etc.) for the purpose of supporting attendance and education.
  • Compiles documentation for the purpose of engaging with students and families.
  • Coordinates activities with a variety of outside service agencies and school sites (e.g. colleges, financial aid, worksource, etc.) for the purpose of assisting parents of eligible children to gain access to needed services.
  • Distributes materials to schools and families for the purpose of providing information on available programs and services.
  • Engages students for the purpose of educational progress.
  • Maintains a variety of documents, files, and records for the purpose of documenting activities, complying with federal, state, and school system requirements.
  • Manages a case load of up to 75 students for the purpose of academic progress toward a credential (high school diploma or equivalent).
  • Participates in a variety of meetings as assigned (e.g. seminars, staff meetings, workshops, etc.) for the purpose of learning,conveying and gathering information required to perform functions.
  • Prepares a variety of materials (e.g. correspondence, forms, monthly mailings, narrative reports, etc.) for the purpose of providing information and written documentation.
  • Presents a variety of information (e.g. community group workshops, parents, etc.) for the purpose of providing information to interested parties.
  • Responds to inquiries (e.g. parents, staff, clinicians, the public, etc.) for the purpose of providing information on Gravity and community resources.
  • Reviews individual needs assessment (e.g. case management for identified students, etc.) for the purpose of overcoming barriers to learning.

Marginal Functions

  • Assists other personnel as may be required for the purpose of supporting them in the completion of work activities.
  • Performs other related duties as assigned for the purpose of ensuring the efficient and effective functioning of the work unit.

Skills, Knowledge and Abilities

SKILLS are required to perform multiple tasks with a need to occasionally upgrade skills in order to meet changing job conditions. Specific skill based competencies required to satisfactorily perform the functions of the job include: applying assessment instruments; operating standard office equipment including using pertinent software applications; preparing and maintaining accurate records; and effective oral and written communication with a variety of social and ethnic groups.

KNOWLEDGE is required to perform basic math, including calculations using fractions, percents, and/or ratios; read technical information, compose a variety of documents, and/or facilitate group discussions; and analyze situations to define issues and draw conclusions. Specific knowledge based competencies required to satisfactorily perform the functions of the job include: local agencies, resources, systems and access to services, District services and programs.

ABILITY is required to schedule a number of activities, meetings, and/or events; often gather, collate, and/or classify data; and use job-related equipment. Flexibility is required to work with others in a wide variety of circumstances; analyze data utilizing defined but different processes; and operate equipment using standardized methods. Responsibility

Responsibilities include: working under limited supervision following standardized practices and/or methods; leading, guiding, and/or coordinating others; operating within a defined budget. Work Environment

The usual and customary methods of performing the job's functions require the following physical demands: occasional lifting, carrying, pushing, and/or pulling, some stooping, kneeling, crouching, and/or crawling and significant fine finger dexterity. Generally the job requires 60% sitting, 30% walking, and 10% standing. This job is performed in a generally clean and healthy environment.

Experience: Job related experience with increasing levels of responsibility is desired.

Education (Minimum): High school diploma or equivalent.

Equivalency: BA Degree in Educational Social Work, Counseling or related field is required or at least two years of work experience providing case management, counseling, or related direct services to at risk individuals or 16-21 year old youth.
