Coordinator of Special Programs Health Services II, Developmental Disabilities

Main Purpose Of Job

The main purpose of this position is to provide targeted case management services to eligible individuals with Developmental Disabilities who live in Cecil, Harford and Kent counties. This position will provide referrals for services, facilitate the development of the annual Person-Centered Plan, monitor individuals in service and on the Eligibility Waiting List, and assist in transition planning. These efforts work to maintain the individual in the community and to promote individual choice and enhance overall quality of life.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The preferred candidate should possess the following:

  • Multitasking and meeting multiple deadlines.
  • Experience in the developmentally disabled population.
  • Being organized with attention to detail.
  • Bachelor's degree in human services field.
