Community Health Worker (Full Time-Edison)

OVERVIEW

Our team members are the heart of what makes us better.

At Hackensack Meridian Health we help our patients live better, healthier lives — and we help one another to succeed. With a culture rooted in connection and collaboration, our employees are team members. Here, competitive benefits are just the beginning. It’s also about how we support one another and how we show up for our community.

Together, we keep getting better - advancing our mission to transform healthcare and serve as a leader of positive change.

Community-Health Workers (CHWs) provide social support, navigation, and advocacy to high-risk low-income patients within the communities they serve in order to help them reach their health goals. CHWs act as caring neighbors to help patients address the social, medical and community problems that lead to poor health. The goal is to help patients deal with the `real life' issues that keep them from staying healthy, help with the tasks of getting medical care, and work on health goals by doing things like planning healthy meals or finding time to exercise.
